To apply for UPMC for You, you must qualify for Medical Assistance (Medicaid) through the state of Pennsylvania. The Department of Human Services (DHS) will determine whether you qualify for Medicaid and what benefits you and/or your family members will receive.

How to apply to Medicaid

Learn more about your eligibility to determine if you are eligible for Medicaid. You can apply for Medicaid through COMPASS or your local County Assistance Office.

COMPASS

COMPASS is an online tool for Pennsylvanians where you can apply for health and human service programs and manage benefit information.

County Assistance Office

Download an application to send to your local County Assistance Office (CAO). You can also contact your CAO for instructions on how to apply. Caseworkers are available at the CAO to help you and answer your questions. They can talk with you in person or over the phone (if the office is closed to the public).

Free Transportation Limitations

UPMC for You’s transportation benefit provides non-medical fixed route transportation and is available to members who reside in the Northeast and Southeast Physical HealthChoices zones only. The counties included in these zones are Bradford, Bucks, Carbon, Centre, Chester, Clinton, Columbia, Delaware, Juniata, Lackawanna, Luzerne, Lycoming, Mifflin, Monroe, Montgomery, Montour, Northumberland, Philadelphia, Pike, Schuylkill, Snyder, Sullivan, Susquehanna, Tioga, Union, Wayne, and Wyoming. The benefit is available at no additional cost. Mobile and paper ticketing options vary by county. If you are a UPMC for You member residing in the Northeast and Southeast Physical HealthChoices zones and have questions about how to utilize the benefit, please contact the UPMC for You Transportation Unit at 1-833-779-6827 (TTY: 711). The Transportation Unit is available Monday through Friday 8 a.m. to 5 p.m.
